ext/ogg/: rework the muxing algorithm.

rework the muxing algorithm.
mux pages based on the timestamp that matches the granulepos of
that page, which is also the granulepos/timestamp of the last
packet that finishes on that page.
rework logging so it's more readable and comparable to oggzdump.
I can't get oggz-validate to complain anymore now.
